when I insert a form from the "form" button into an article, it gives me the following error.

0 - Call to undefined method JModuleHelper :: getModuleById ()

if I change the template, I view the page well and without error.
so the problem is in the jd_chicago template.
do you have any solutions?

Are you running the latest version of Joomla? If no, please update to the latest one.
Please enable debug and error reporting to see what file is having the issue. It looks like this is something that got changed around 3.9.0 and your template/plugin is still referring to the old code reference for loading modules.
